Insurance
ª Dr. Tookes accepts and participates with most insurance plans.
ª Please call us or your health plan to verify participation with your plan.
ª A referral from your primary care provider may be required.

Payment Options
ª Insurance copays and non-covered items payments are expected at the time of your visit.
ª Payment methods accepted include: cash, check, Visa, Mastercard and Discover.
ª A discount is offered for those non-insured patients who pay in full at the time of visit.

Cancellation and No Show Policy
ª 24-hour notice of cancellation is appreciated.

ª A $25.00 fee maybe added to your account for no show appointments
